TEAMCARE and COMFORTage workshop report from Radical Health Festival Helsinki
The TEAMCARE and COMFORTage projects have published the report from their joint workshop held on 20 January 2026 during the Radical Health Festival in Helsinki. The workshop, titled “Digital Transformation in Action,” brought together European experts to discuss how training, collaboration and digital tools can support the implementation of integrated, person-centred care.
The report presents key insights from TEAMCARE pilots on interprofessional training for community-based care, as well as COMFORTage activities focused on strengthening digital health literacy and supporting the prevention and management of dementia and frailty through digital solutions and community engagement.
